Information

Pursuant to the Article 22 of the Act of December 15, 2017, on insurance distribution, we present the required information about us as the insurance intermediary:

• Quadra Underwriting sp. z o.o. is an insurance agent representing more than one insurer;

• Quadra Underwriting sp. z o.o. provides the client with the power of attorney granted by the insurer during the first action falling within the scope of agency activity;

• The headquarters of Quadra Underwriting sp. z o.o. is located in Warsaw at ul. Ogrodowa 31/54, 00-893 Warsaw;

• Quadra Underwriting sp. z o.o. is registered in the register held by the Polish Financial Supervision Authority under number 11251448/A, which can be checked by entering this number or company name at https://rpu.knf.gov.pl/search/agent;

• Quadra Underwriting sp. z o.o. receives, from the insurer on behalf of whom it concludes the insurance contract, commission included in the amount of the insurance premium;

• Quadra Underwriting sp. z o.o. does not deal with the distribution of consumer insurance products or the distribution of life insurance, referred to in Group 3 of Section I of the Annex to the Act of 11/09/2015 on insurance and reinsurance activities.

 

Claims and Complaints

The client may submit a claim under the insurance contract concluded through us directly to the insurer or to the e-mail address contact@quadra.com.pl.


The client has the right to lodge a complaint. Complaints may be submitted by e-mail to the e-mail address contact@quadra.com.pl, as well as in writing (in person) at the registered office of Quadra Underwriting sp. z o.o. or by post within the meaning of Article 3 Point 21 of the Act of 23 November, 2012 - Postal Law, or orally by phone or in person [for the record] during a visit to the headquarters of Quadra Underwriting sp. z o.o.


Complaints about the services provided by Quadra Underwriting sp. z o.o. (i.e., complaints not related to the insurance coverage provided) are considered by Quadra Underwriting sp. z o.o.

 

Complaints related to the insurance coverage provided, filed to Quadra Underwriting sp. z o.o., for which the insurer is competent to recognise, will be immediately forwarded by Quadra Underwriting sp. z o.o. for consideration by the insurer.

Quadra Underwriting sp. z o.o. will respond to a complaint unrelated to the insurance coverage provided within 30 days of its receipt. To meet the deadline, it is enough to send a reply before the deadline. In particularly complicated cases that make it impossible to recognise a complaint and provide a reply within the time limit referred to above, Quadra Underwriting sp. z o.o. will explain the reason for the delay in the information provided to the client who lodged the complaint, indicate the circumstances that must be determined for the consideration of the case and specify the expected deadline for considering the complaint and responding, which may not exceed 60 days from the date of receipt of the complaint.

 

If the claims arising from the complaint are not considered, the client – who is a natural person – has the right to submit a request for consideration of the case to the Financial Ombudsman at the following address:
